Keratolysis Exfoliativa

Keratolysis Exfoliativa

Keratolysis exfoliativa (also called exfoliative keratolysis or dyshidrosis lamellosa sicca) is a common, benign skin condition characterized by peeling or superficial blistering of the palms of the hands (sometimes soles of the feet).

It’s a non-inflammatory skin disorder where the outer layer of the skin (stratum corneum) peels off in small sheets or rings.  It’s often mistaken for eczema or contact dermatitis, but it’s typically less itchy and doesn’t involve inflammation.

What Does It Look Like?

  • Dry, peeling skin, especially on the fingertips or palms / soles

  • Air-filled blisters or small clear vesicles that peel away

  • Usually no redness, itchiness or pain

  • May be seasonal, often worse in summer

  • Can affect both hands/feet or just a few fingers/toes

What Causes It?

The exact cause is unknown, but it’s believed to be linked to:

  • Frequent handwashing or use of irritants (soaps, sanitizers)

  • Heat, humidity and sweat

  • Friction (e.g., manual labor or frequent contact with surfaces)

  • Possibly a genetic predisposition

It’s not contagious and not caused by infection (unlike tinea or bacterial conditions).

Treatment & Management

There’s no “cure,” but it’s manageable with some simple steps:

Do:

  • Use mild, fragrance-free moisturizers (especially after handwashing)

  • Apply barrier creams to protect from irritants

  • Use gentle cleansers or soap substitutes

  • Limit contact with harsh soaps or detergents

Avoid:

  • Over-washing or using alcohol-based sanitizers excessively

  • Scrubbing the peeling skin (this worsens it)

  • Prolonged exposure to water (e.g., dishwashing without gloves)

In more severe or chronic cases, dermatologists may prescribe:

  • Topical corticosteroids (short-term)

  • Urea or lactic acid creams to help exfoliate gently

We are independent prescribers able to prescribe any medication from the BNF relevant to the lower limb, foot and ankle.
