Painful, Curved Toenails: Managing Pincer Nails with Precision
If you’ve ever noticed your toenails curving sharply inward at the sides almost like they’re pinching the skin beneath them — you may be dealing with a condition called pincer nails.
They are also known as trumpet nails or omega nails and most commonly affecting the big toenails. Over time, this pinching can cause pain, pressure and tissue damage.
Some of the causes of pincer nails include too tight or narrow shoes, trauma or injury to the nail or repeated stress, fungal infections and genetics.
Treatment Options for Pincer Nails
There are multiple ways to treat pincer nails, depending on the severity of the condition.
1. Conservative Treatments
Best for mild to moderate cases:
Nail Bracing
-
Small braces or wires are applied across the surface of the nail to gradually straighten it.
-
3TO nail bracing is a recognized and effective treatment option for pincer nails, especially in mild to moderate cases
-
Great non-surgical option for many patients.
Regular Nail Debridement
-
A podiatrist can trim and thin the nail to reduce pressure.
-
May need to be repeated regularly for maintenance.
Footwear Adjustments
-
Wear shoes with a wide toe box and avoid high heels or narrow shoes.
-
Reduces chronic pressure on the nails.
Antifungal Treatment (if needed)
-
If the nail is thickened due to fungus, topical or oral antifungals may be required.
2. Surgical Treatments
For more advanced or painful cases:
Partial Nail Avulsion
-
Removal of one or both sides of the nail under local anesthesia.
-
Can include phenolization to prevent regrowth of the curved portion.
Total Nail Avulsion
-
In severe or recurrent cases, full removal of the nail may be necessary.
This painful nail deformity is more common than most people realize and can lead to serious discomfort or even infection if left untreated.
