What is Pustular Psoriasis?

Pustular psoriasis is a rare and severe form of psoriasis characterized by white pustules (blisters of non-infectious pus) surrounded by red, inflamed skin.  It can appear in localized areas such as the palms and soles or cover large portions of the body.

Types of Pustular Psoriasis

  1. Generalized pustular psoriasis (GPP).  A serious and potentially life-threatening form that causes widespread pustules, fever, chills, fatigue and systemic inflammation.  It often requires urgent medical treatment.
  2. Palmoplantar pustulosis (PPP).  A chronic form affecting the palms of the hands and soles of the feet.
  3. Acrodermatitis continua of Hallopeau.  A rare form affecting the fingers and toes, often involving the nails.

Causes and Triggers

While the exact cause is unknown, pustular psoriasis is believed to result from an overactive immune response, where the body mistakenly attacks healthy skin cells.

Potential Triggers Include:

  • Infections (e.g., upper respiratory infections)
  • Certain medications (e.g., corticosteroid withdrawal, lithium, NSAIDs, beta-blockers)
  • Stress
  • Smoking
  • Pregnancy (can trigger a rare type of GPP called impetigo herpetiformis)
  • Genetic mutations (e.g., IL36RN gene in some GPP cases)

Symptoms of Pustular Psoriasis

Depending on the type, symptoms may include:

General Symptoms:

  • Clusters of white pustules on red skin
  • Pustules may dry and peel, leading to scaling
  • Severe itching, burning, or pain
  • Fatigue, muscle weakness

Systemic Symptoms (GPP):

  • Fever and chills
  • Headache
  • Nausea
  • Rapid heart rate
  • Dehydration

Symptoms can range from mild and localized to severe and widespread, particularly in GPP.
