Syphilitic paronychia

 

Syphilitic paronychia, though rare, is a manifestation of syphilis, a sexually transmitted infection caused by the bacterium Treponema pallidum. Syphilis can have various clinical presentations, and when it affects the nail, it is referred to as syphilitic paronychia.

Causes and Pathophysiology

  • Primary Syphilis: In the primary stage, syphilis typically presents as a painless sore (chancre) at the site of infection. If this occurs near the nail, it may lead to syphilitic paronychia.
  • Secondary Syphilis: During the secondary stage, syphilis can cause widespread symptoms, including skin rashes and mucous membrane lesions. Secondary syphilis can sometimes involve the nails, leading to paronychia.

Symptoms of Syphilitic paronychia

The symptoms may include:

  • Pain and Tenderness: Around the nail bed.
  • Swelling and Redness: In the affected area.
  • Nail Changes: Possible deformities or discoloration of the nail.
  • Pus Formation: Though less common, secondary bacterial infection can lead to pus.

Treatment of Syphilitic paronychia

Treatment for this condition is the same as for syphilis and typically involves:

  • Antibiotics: Penicillin G is the drug of choice for treating syphilis. For those allergic to penicillin, alternatives such as doxycycline or azithromycin may be used.
  • Symptomatic Treatment: Managing pain and inflammation with analgesics and anti-inflammatory medications.
