The Weird and Wonderful 🌴
Today we will discuss rare cases. Understanding rare diseases is essential for improving early diagnosis, treatment access, and patient outcomes.
What is Mycetoma?
This is a chronic, progressively destructive infection of the skin, subcutaneous tissue and sometimes bone. It commonly affects the foot, earning the nickname “Madura foot”. It is characterized by swelling (tumefaction), multiple draining sinuses, and discharge containing grains.
This condition is common in tropical and subtropical regions and often affects farmers and rural workers who walk barefoot.
Organisms enter through traumatic implantation (thorn prick, splinter) and slowly spreads through:
-
-
Subcutaneous tissue
-
Muscle
-
Bone (late stage → deformity)
-
Unlike many infections, pain is often minimal initially.
Leishmaniasis (“Leish”)
This is a parasitic disease caused by protozoa of the genus Leishmania, transmitted by the bite of infected female sandflies.
It mainly affects the skin, mucous membranes or internal organs, depending on the species.
Symptoms include: painless papule → ulcer with raised border, “Volcano-like” lesion, prolonged fever, massive splenomegaly, weight loss, anemia & pancytopenia, and darkening of skin.
Chromoblastomycosis
It’s most common among:
- Farmers
- Agricultural workers
- People who walk barefoot in rural areas
- Basically, it’s the ultimate “occupational hazard” fungus.
Sporotrichosis (Rose Gardener’s Disease)
This is a chronic subcutaneous fungal infection caused by Sporothrix schenckii, a dimorphic fungus found in soil, plants and decaying vegetation.
Infection occurs through traumatic implantation such as thorn prick, splinter, or handling soil or moss.
This usually affects:
- Gardeners
- Farmers
- Florists
- Forestry workers
