Onychomatricoma
Onychomatricoma is fortunately not common. It is a rare, benign tumor of the nail matrix—the part of the nail bed that produces the nail plate. It’s not cancerous, but it can cause distinct nail changes that make it look unusual or even alarming.
It is a fibroepithelial tumor arising from the nail matrix. It has both epithelial and mesenchymal components, which is why it’s sometimes referred to as a biphasic tumor. This condition is slightly more common in women and typically found in middle-aged adults. The exact cause is unknown but trauma might be a trigger in some cases.
They most commonly affects a single fingernail, especially the thumb, but can also appear on toenails.
Typical Signs of Onychomatricoma
- Thickened nail (often yellowish or brownish)
- Longitudinal ridging or a band running down the nail
- Splitting or over-curving of the nail
- “Worm-like” projections visible under the nail (on dermoscopy or histology)
- Sometimes, blood or pus-like discharge from under the nail
- Usually affects one finger, especially the thumb
Diagnosis of Onychomatricoma
- It is often confirmed with dermoscopy, ultrasound or biopsy
- Chameleon like it may be mistaken for:
- Fungal infection
- Subungual melanoma
- Nail psoriasis
- Onychomycosis
