When Nails Speak: Recognizing Periungual Fibroma

One minute read

If you’ve ever noticed a small, firm growth near the edge of your fingernail or toenail, you’re not alone.  One possible cause could be a periungual fibroma—a benign growth that can appear in the skin around the nails.  While it might sound alarming, it’s usually harmless.  However, in some cases, it can be a sign of an underlying medical condition that needs attention.

What Exactly Is a Periungual Fibroma?

A periungual fibroma—also known as a Koenen’s tumor—is a non-cancerous, fibrous skin tumor that develops near or under the nails.  These growths tend to be:

  • Small (a few millimeters in size)
  • Flesh-colored or reddish
  • Firm to the touch
  • Occasionally painful or irritating, especially if they affect nail growth

They’re most commonly found on the toes, particularly the big toe, but can appear on fingers as well.  They usually emerge during late childhood or adolescence and may grow slowly over time.

Why Do Periungual Fibromas Occur?

While anyone can develop a periungual fibroma, multiple fibromas are often linked to a genetic condition called Tuberous Sclerosis Complex (TSC).  This is a rare disorder that causes benign tumors to grow in multiple parts of the body, including the brain, skin, kidneys, heart and lungs.

In fact, periungual fibromas are considered one of the diagnostic markers for TSC, especially if they appear during late childhood or early adulthood.

Treatment Options

  • Observation: If the fibroma is small and asymptomatic.
  • Surgical removal: For cosmetic reasons or if it causes pain, nail deformity, or recurrent trauma.
  • Laser therapy: An alternative to surgical excision.
